Steps to Cancel Classes

  1. Fill out the form below.
  2. We will use the date you submit this form as the start of your 30 day notice.
  3. If unenrolling mid month we will prorate your final month's tuition.
  4. If you have any make up classes that are owed to you please schedule them before your membership ends. Any make up classes still left on your account after your membership ends will be forfeited.
  5. You will receive a cancellation confirmation email within 48hrs of submitting this form. This email will include your final tuition due, and final day of class.
  6. Be sure info@smallfrydanceclub.com is not being blocked by your spam filter.
